Volunteer type position is suitable for:
  • Adult (18 +)
Duties / Roles / Activities: - Visits clients in their homes to conduct wellness assessment surveys/questionnaires
- Assists clients in personal health-related goal identification
- Provides social connection to older adults
- Communicates with volunteer organizations, family health team, research team, and/or community based organizations
- Utilizes a tablet computer and other technology to provide information to identified organizations
- Maintains up to date knowledge of health programs and services and acts as resource in the community through the use of approved information and referral sources
- Works closely with program coordinator to schedule client appointments
- Completes feedback and reports; and submits information to the volunteer coordinator
Sample Tasks: - Visit clients at their home (average visit time from 1 to 2 hours) - the conditions of the client’s home may not be known prior to the visit occurring
- All visits will be within the assigned community
- Local travel is required. Must have reliable transportation by personal or public transit (mileage and transit costs reimbursed)
- Meetings and supervision may be in person, by telephone or through other electronic means
- Must have access to a telephone, email and internet
- Time spent engaging in activities will vary by a volunteer’s interest and availability with the average person committing 6 hours per month
- Required to take online and face-to-face training
- Will be required to take tuberculosis test (reimbursements available)
